Save Files Not Saving Properly

For the last couple weeks or so, my compositions have not been saving properly sometimes. When this happens, they either create a new save file with the changes, or most recently, did not save any of the changes I made. I am neurotic about saving every time I make a change, so I know this is not a mistake on my part. Is anyone else running into this issue?
