WHY IS THIS HAPPENING?

Why is this happening? The slash marks in the measure are right on the beat. The cued in quarter notes on beats 1 and 2, above the slash marks in the measures, are not exactly over the slash marks and they should be. They appear to be on the "and" of beats 1 and 2.
Also, the two eight notes on beat 3 are squashed together which is not correct. Don't know why this is happening. I've had this happen before and I've done it the correct way too.

It is obvious from your photo that the top voice is voice 2.
Select the bar(s) and press [shift]v to swap the voices, then come back if this doesn't work.
