ok, so, it looks like I have to delete everything "realize chord symbols" has done, and then re-run it, any time I make a change to the chord symbols -- is this correct?
I'm wondering if sibelius can't just regenerate the chord symbol realizations any time I make a change?

1. you only need to delete the ones you've changed and only apply the plugin to whole bars.
2. If you want to undo the chord realisation, use [ctrl]z.

Yes it can be a pain but there's no dynamic interaction. It's simply a plugin you run to create the notes.
I normally keep a version without chord realisation (just chord symbols), as well a realised version. If a chord needs changing you can change it on the unrealised version, re-run the plugin and copy/paste, either the whole thing or just the affected bar(s).
